If you have a two story house, my answer is a pack n play with a changing table. It's honestly a godsend for us because I don't have to run up and down the stairs all day long. Plus, I found with my daughter that napping her in the pack n play all day helped her realize the difference between night and day.
Another thing is zip-up one piece pajamas for middle of the night changings. It makes life SO much easier when you're tired and barely awake to just zip up the pjs rather than fuss with the snaps.

I can't just post one, here are my top three:

Bouncy Seat with womb sounds and vibration - She lived in this thing until about 7 or 8 months old.

Sanp and Go - The easiest way to go to the store or run any quick errands. Never had to wake the baby to go somewhere and it is so compact.

Angel Care Monitor - I never thought I would be but when she came home from the hospital I was a wreck. I must have checked her breathing a million times the first two or three nights. I broke down and purchased the Angel Care and slept as soundly as you can with a newborn..

- Hands free BFing bra (life saver if you plan to BF)
- Good quality double pump
- PNP
- Stroller than reclined to flat position (we did a lot of walks when DS was born. I would just put him in a laying down position and we would be cruising the neighborhood).
- Infant carseat (so much easier to take child in and out of car).

WISH I had gotten earlier. Those huge shade to extend the canopy of your stroller. Especially if you have a baby in the Summer and walk outside.

the right swaddling blankets - I went through a lot of gimmicky swaddling things, the only thing that worked for us was large flannel blankets by swaddledesigns and a good technique!

I'm writing stuff that initially were lifesavers....after a few months, you can try out different things...but the first few months, these were lifesavers (we still use the snap n go at times at 10 months)

Also, burp cloths!!! I used cloth diapers for this but they were essential..my DD was a huge spitter upper

oh miracle blanket also...I absolutely give it credit for helping DD sleep well at night as a newborn.
