I say, "No, they checked, there's only one in there. I have narrow hips and no torso so the only place for baby to go is outward."

Yes, my Dr who has over 30 yrs of experience and, unlike you, HAS seen the inside of my uterus is quite sure, thanks!
ETA..and I'd probably lift a leg and be like "wanna check"

How would YOU respond to this question....
If I didn't know them well/like them, I think I would put them on the spot and say: "yes, I'm sure. Why, do you think I look abnormally large for a pregnant woman?"
For some people, I would laugh with them. I was joking with my mom when I felt huge with my second and she said that to me, and I was laughing with her saying who knows, it sure feels like it.

to a complete stranger my reply was "Nope, there is just one." While I laughed and secretly imagined smacking her.
to my MIL, the 1st few times: see above. After the 4th or 5th time she joked about it, I said, "No Mom, there is only 1 baby in there my dr is completely competent and able to read a sono. As I've already told you, my stomach is large b/c I am on a high dosage of progesterone, which causes me to bloat and I am on such a high dosage b/c I don't want to have ANOTHER miscarriage!!!!!!!!!!!!!" 
That shut her trap on that subject.
