I had the same question
here is the link for more info and lawyers website. It covers about 30 different med plans that have united healthcare, you need to print the form and fill it out. Mail back by Oct 5th.

I just filled it out a few seconds ago..thanks to DH telling me about it at 9am.
As the subscriber, we had to complete
Section #1 on page 10
Section #2 on page 11
Section #6 on page 15.

I believe it's Empire BC/BS. DH works for the lirr and he got the info from his union.
