Another disability question- who has to fill out the paperwork?
1- Who provides you with the paperwork? Does your employee have to get it or do we have to get it on your own?
2- Who has to fill it out?

I think my employer gave me the paperwork then I dripped it at my drs and they called when I could pick it up.

Your employer gives you the paperwork. You fill a portion out, your doctor does and then you return it to your employer to complete and send in.

My disability company sent the paperwork to my Dr and they fill it out.
