Disability paperwork question...what is the BC/BS voucher?
I had some issues with my NYS disability, apparently my company filed it wrong or something and it delayed me getting it. Anyway I corrected the problem and recently received my retro check, but there was paperwork that came with it that I have to fill out and send back to them in order to get the final 3 weeks paid. It is asking for the info to be filled out by the hospital if I can't provide the BC/BS voucher.....I have no idea what that is!! Can anyone help?  TIA

its a blue cross/blue shield voucher. I dont know what it means in terms of your disability though, but I think thats what it is.

Thanks! So basically since I don't have blue cross blue shield I have to get the hospital to fill it out.....thats annoying, they give you a weeks time to do it, like that's gonna happen!
