Transition to milk
When and how did you start the transition from breast milk or formila to whole milk? I plan to mix breast milk and whole milk together but I'm not if I should start at 12 months or a couple of weeks earlier so the transition is complete at 12 months. Also what ratio milk to breast milk/formula did you use and how long at each ratio.

Transition to milk
It depends on the child. 12 months is not a magic number, just an estimate of when they've moved on to a variety of foods. If your child is eating a good amount of fruits, veggies, proteins, while grains, etc. then you can probably start. Also keep in mind if your child is drinking 24oz of BM/formula, that shouldn't translate to 24oz if milk. Milk is just a drink, and they shouldn't be filling up on it. It doesn't provide the same nutrition.

Thank you. Super helpful. I forgot to ask. Did you heat it?

Thank you. DD2 is a bottomless pit. She prefers solids of any kind to milk. She probably is only drinking about 12 oz a day now at 10 months. Her pediatrician recommended only doing one 5oz bottle a day and to start with 1oz milk and 4oz breastmilk I just forgot to ask when to start it. I plan to continue breastfeeding 1x am and 1x pm as well.

Same here, but breast milk to cow's milk. It was easy, she never looked back.
ETA: I did warm it a little at first, but less and less. Once it was mostly cow's milk, not at all.
