when did you give your dr the paperwork?

my benefits/HR dept told me to come back in June to pick up all the paperwork my dr needs to fill out. i'm due in July and my last day of work will be July 1st. when did you give your dr the paperwork to fill out? does he do it right in front of you at one of your appts or do you usually have to leave it with him and then pick it up another day?

I would call your dr.'s office and ask. I will be going out of work early (doctor's orders) and the nurse told me to get all the papers I need from HR and bring them to the office at least 2 weeks prior to my last day. That I would leave them and they would call me when they are ready and either I could pick them up or they could mail them. But, she did say it takes about 2 weeks. I think every office is different though.

I asked my HR rep yesterday and she said 30 days prior to going out on maternity leave. I felt that was waiting a little too late as well.

thanks. i have another appt next wed, so i'll get the paperwork before then and bring it with me to my appt.
