I have. Most of the supermarkets go to are by work. I look at the receipt before I leave the parking lot. Otherwise I am too lazy.

I have returned meat that when I opened the package was spoiled. I call first to let the store know. In one case, the case had apparently gone bad. They thanked me for letting them know so they could pull the stock. I have returned bread that had packaging baked inside of the loaf. I have returned a salad that had a frog inside of it. That one was gross and bad.

I once returned $3 strawberries out of principle. Probably was not a good use of my time as my time was worth more than the time I wasted trying to get a $3 refund.

This is crazy that you posted this! I returned something today. I ordered roast beef at the deli counter... new guy, and I asked for the sale roast beef, whatever it was. I was shown a really nice piece and just said yes. I was distracted by my son, but thought about it when I got home, "that looked like the London broil". I almost fell over! $16.99/lb! 
About a month ago I got strawberries at Wild by Nature. I accidentally took the organic instead of conventional. $12! it was the first time I returned something. The lady told me I was the 4th person to return those this week
