I saw her in the Hamptons on 2 separate occasions, and she looked completely cracked out/on something. And one of those times was at about 1 in the afternoon! That's what years of partying will do to a person.

This is confusing me too. I know make-up can do a lot but that's a BIG difference. I hope for her sake that she doesn't really look that awful in real life.
I definitely do think she still hits the bottle pretty hard and/or does drugs. I saw her interviewed sorta recently and she was saying that she cleaned up her act but "still likes to have fun" or something along those lines. It basically translated that she still parties a ton but maybe only 5 days a week instead of 7.

That's 100% her. Her eyes are puffy, no make-up, her hair is a mess and its shadows are making it look like her forehead is more wrinkled than it actually is, though she's also being blinded by the sun so her face is scrunched up. She's probably just waking up from a rough night. It's amazing what a good stylist can do.

The wrong lighting, no makeup or hair done can easily accentuate all the worst. She is squinting in the sun, has messy hair and no makeup. Thats what I see.

not even sure why she had stomach surgery b/c she was always skinny from what I remember.
