For your birthday
Does DH plan what you do, or do you have to make all the plans? Mine is Monday and we have off, but I'm afraid if I leave the plans to DH, we won't do anything

If I know there is something specific that I want to do I will generally let him know, that way I'm not disappointed. He's not a mind reader and I really like my birthday! 
Just let him know you would like to do something or give him an idea and let him take it from there.

Some years he plans, other years I say if there is something specific I want.
One year he planned Les Mis tickets on my actual bday and it was one of the best bdays ever!

I usually make the plans. I share a bday with my cousin and we hang out with the same group of friends, so, I usually host something here the weekend before, or after.
On my actual bday, I still make the plans. LOL.

DH will usually ask if there is anything special I would like to do before my birthday arrives. Or if he thinks I may enjoy something - he at least asks if it is something I would like to do.
I have no problem though suggesting what I want to do before he says anything.

B-day's are no big deal to us.
We go to dinner- he gets me a nice card and some gifts.

When my birthday is on a weekend I like to spend it at the pool. That is the plus of a summer birthday, no planning just tanning! 
I plan DH's birthday for him. It will involve a cake and a card.

That said, once he asks me, I usually end up making the plans anyway. I'm better at it.

Well, I subtly hinted for months where I wanted to go and what I wanted to do But the weekend was great and extra special because we attended one of his friends weddings upstate.

Okay, so a lot of your DHs don't surprise you with outings on your birthday. When I said that if I left it up to him we would not do anything, that was an exaggeration. It just means he would ask me what I wanted to do and we would do that.
But I love surprises and I wish my DH would surprise me more.
