Diagnosis
Diagnosis is that its immunological and it is treatable with Lovenox and Neupogen (if insurance doesn’t cover it then id have to take Prednisone because it would be 3500 for one cycle of Neupogen)…
There are a few causes… we have 7 HLA matches… The body’s natural reaction is stop an embryo that has too many HLA matches. Dr B said hes rarely sees this many matches.
NKA (natural killer activity)– 30 – Dr B said it should be below 20… so its not borderline its way over the line…. & I have hyperthyroidism. I am making an appointment with Dr Bs brother in law Dr Barandes to get started on L-thyroxine…
this kind of explains the NKA stuff in relation to fertility Hyperprolactinemia is often an early indicator of impending or existing thyroid hormone deficiency or hypothyroidism (Hashimoto's disease) which in most cases results from antithyroglobulin and/or antimicrosomal antibodies that attack thyroid hormone producing glands, replacing them with connective tissue. In about 50% of cases where the woman has such thyroid antibodies in her blood (regardless of whether or not they have concomitant hormonal or clinical evidence of thyroid deficiency) she will also have increased natural killer cells activity(NKa). When this happens embryo implantation will likelyb be impaired and the woman will often present with inability to conceive ("infertility"), unexplained IVF failure, or with recurrent pregnancy loss (RPL).
And he said something about Protein 2, but that’s when I stopped writing what he was saying… Im going to get the entire report faxed to me tomorrow so I can read it myself… I update tomorrow when I get the report myself.
He told me to start taking some vitamins… a Max Formula Antioxidant and 4000mg of Omega 3 everyday.
We are NOT starting this cycle because we have to get my thyroid under control first...

Re: Diagnosis
I have high NK levels as well as a few other things which I have no idea what they are.
Honestly, it's better to be where you are right now- you went to the right person for the issues you have and he's going to treat it properly. The way he explained it to me is that my high NK levels essentially kill off any foreign substances in my body, zygote/fetus included.
Did he mention anything about intralipids? The meds arent that bad. As much as I bitched about Lovenox, I'd do anything to be filled with rashes and pregnant right now! (The reasons for my mc were likely chromosonal. I just got my immune testing back and it was all normal). I was the 1 girl in a zillion who got a reaction from the Lovenox, so dont worry. But the fact that I got pregnant after those treatments is just a testament to the treatment working! Good luck!!!

Re: Diagnosis
Dr. B figured out I have Hashimoto's disease (underactive thyroid), I met with 2 endo's before finally meeting with his BIL. The other 2 endos I didn't like and after meeting with his BIL I was like omg I LOVE him!
He is so bubbly and chipper and nice. He doesn't have a nurse and took my blood himself! He even prescribed me meds at 100mg instead of 50mg so I can snap them in half and a 90 day supply turns into a 180 day supply.
Good luck! Hopefully this fixes the problem!
And wanted to add it took me like 2 weeks on meds to get my thyroid levels normal.

Re: Diagnosis
wow 7 HLA matches.. thats crazy!! def explains a lot on its own! DH and I have a partial HLA-C match. i also have elevated NK cells and a PAI-1 4G/5G polymorphism (blood clotting issue), oh yeah and of course PCOS which makes me only semi-ovulatory
i was treated with intralipids for the NK, lovenox for the clotting and the HLA match, and baby aspirin too. everything else was for PCOS/ovulation induction. with dr. B i got pregnant twice with ovulation induction alone, but both were chemical. thats when we did the thrombo and immune panel which showed the rest of the issues. we treated all of them and didn't get pregnant for two cycles, and eventually moved to IVF (due to PCOS) and i got pregnant on the first try and now have a 4 month old DD (if you're bored and want to read that story to see the protocol, here's my IVF story ). the blood clotting issues lead to pre-eclampsia at 32w and the pregnancy ended crazily (im sure you've seen my posts about it, but if not here's Emily's birth story ), but in the end she is here and I would do it all again exactly the same if it meant i would end up with my princess
if you have any questions or need anything along the way, please FM. i know how overwhelming it can be to get these diagnoses, but i promise you it is better to know whats going on and to treat it, than to not know and continue to suffer loss. dr. B is my miracle worker, issues like yours are really his specialty much more than basic reproductive endo. you are in good hands!!

Honestly I didn't feel like the thyroid meds made me feel much different , maybe just a bit (went on in April 2011). My TSH level was at 5.5 which is a bit high, in early 2009 (before TTC) I was sick for like 2 months, looking back all the signs of a Thyroid disease were there, but my TSH levels (checked old blood work recently) was 3.5, they say you should treat the symptoms and not the numbers, but that fell into my GP's "normal range" and he didn't pick up on it. It's super important to be on meds even if you don't have sympotoms otherwise your body is just attacking the Thyroid.
BUT, I did start to feel better and GOOD about 2 months ago, after my immune panel came back I had some slightly high or borderline high (but still in the normal range) issues, MTHFR (C and A copy), slightly high NK, Thyroid antibodies were 80...
Anyway Dr. B put me on Resverstrol (antioxidant), Melatonin, Greens Plus, Royal Jelly (super nourishing), Pycnogenol (antioxidant), Metnax (vitamin for the MTHFR), and Intralipids, I am currently on my 3rd month of this and feel a lot better now. So I think these supplements are what made the difference! I love that Dr. B prescribed me some natural remedies.
I would ask him about the Intralipids for you too. You get it once a month before implantation, and again one time when you're pregnant.

I have Hashimoto's too and other than synthroid it was never suggested that I do anything else... they seem to think synthroid "solved" the problem but I am no longer so sure.

I think its just standard procedure along with the blood work to make sure there is nothing in the uterus that should not be!
