A fee to fill out disability paperwork?!
I can not believe my OBGYN charges a fee to fill on NYS Disability Form... a simple 5 question piece of paper with his signature and his office charges a fee.... i am speechless!

that is so not ok... i have given so many forms that are required by my employer to my doctor to fill out... tg she doesn't charge!
