Have you ever requested a particular teacher for your child?
  What grade?
  Was there a specific reason for the request?
  Who did you contact - the teacher or principal?
  Did your child get the requested teacher?

I haven't made a request,  but our school welcomes them, and sends a note home saying to  have our requests in by xxx date

Our district welcomes requests for certain things but NOT for teachers.  If you request a specific teacher you will not get them.  You can tailor your request to give you a shot like my child does best with someone who is warm, or stern, or what every traits you are looking for.  Or I am looking for a similar or different experience than my older child had with x.  You should see if your district has that policy.  I know people request to be with another child or away from another child, to be in allergy classrooms etc.

My school district specifically sends a letter home saying barring an extenuated circumstance that teacher requests will not be considered.

We are not allowed to request teachers. We can send a letting with info about the type of teacher we think the child needs but we can't request by name.

 Requesting a teacher for your child. 
Unless you have an older child who had the teacher before and you had a bad experience, I do not like the requesting of teachers.  What one person thinks didn't work for their child, might work for another.  I would though send in a note requesting a "type" of teacher, strong in math, strong in reading, strict, more warm, etc.

Would you request that your child be placed with a best friend in kindergarten? If you thought it would make the transition easier and you anticipated it would be an issue if he was not in the same class as his BF?

  Honestly, no, and I really don't think a principal would honor this. I think it's reasonable to hope a child might see a familiar face in class, but a best friend? I think that seems unreasonable to ask that of a principal. Unless the child had special needs (forgive me if I missed that part) can you imagine how difficult a job it would be if each child entering K needed to be placed with their best friend?
  Our school you cannot make requests for teachers, or anything, other than to express if you feel your DC will do better with a certain type/style of teacher. For ex, one might say their DC really needs a stern teacher, or a more nurturing teacher.

Honestly, as someone who works in a school...a request like that would be greeted with an eye roll....I would only request for specific reasons like the ones listed above, like bad previous experience with a teacher or because a teacher has an expertise in something.

Can't hurt to ask.
  Ours here will attempt to do so.  Actually her BFF's mom and I were just talking about this a few weeks ago.  They live across the street from each other and had kindergarten together.  It made it easier for her and I to share info on the class, etc.  I'm hoping they have 1st grade together as well.  Guess I should put in a word for that now that you mention it.  Ironically though, we don't care as much about the 'teacher' as having the girls together.  Whereas the other mother cares more about the teacher.  So I figure I'd just let AJ follow her BFF where ever.

  Ours too.  I spoke to his current teacher about a few issues and then wrote a formal request letter.

  No, I wouldn't.  I think in some ways, he would have a better experience in Kindergarten without having his friend in the same class.  
  I am not a fan of requesting teachers unless an older sibling/parent had a bad experience previously and even then, it would be requesting to NOT have a certain teacher.

I just asked about this yesterday my son just finished Kindergarten and I LOVED his teacher so I had asked about when my daughter enters can I request her she said absolutely. The September that she starts I have to put in my request the year before (June) and contact the principal.

I haven't, but on our local mom's group many of the moms do. It also sounds like most of the time they are granted. 
  It seems most request based on their child personality/learning style. They explain Mr./Ms. so and so would be a good fit for their LO based on their particular teaching style. 
  GL!! I was very surprised to see this as a normal practice..
