Family Medical Leave Act/Disability - Paperwork
I'm so confused over this paperwork. My Dr. said that he normally fills it out after the baby arrives. But my job is telling me that there is a 7 day elimination period before disability benefits begin so if I list the due date as the date I can no longer work, I will lose one week of disability benefit payment since disability is paid up to 6 weeks after the birth of your baby for a natural birth and 8 weeks after delivery for a c-section.
What did you guys do?

It's a bit foggy now, but I'm pretty sure that I dropped the papers off to be filled out after I had ds#1...

You have to wait until after you have the baby and put the actual date of delivery. If there is a 7 day waiting period you can just mail the paperwork in during that time. The Dr has to sign the paper with your actual delivery date. A lot of companies just mail you one check for the entire amount of your disability.

Ah - yes! This is exactly how it went for me. I received one check after the 6 weeks was up.

I had my baby last Friday. I just filled out my paper work for short term disability (I don't qualify for FMLA since I am only part time). I am having my OB fill it out this week, and then it goes back to my office to be filed. I am not sure how much I will get as a PT employee, but anything is better than nothing.

I had the paperwork filled out before DS was born and the company that handles my companies disability adjusted accordingly.
For NYS - I just had them sign it and they told me what to fill out - I faxed it back after DS was born.
I have a 5 day (business days) elimination period for my job and got paid for 5 weeks of disability afterwards.

You will lose 1 week anyway as a waiting period for state disability, everyone does.
You must send the paper in after the baby is born so the doctor can sign and fill it out.
Even if you were to send the paper in early and have the doctor put your due date, you will still lose 1 week.

I'm confused. Who gets the paperwork after its filled out? I work for a very small company and I spoke to my boss and she wasn't sure about it. She said that they'd be willing to fill out/send in whatever I need but I'm still lost. I have the form from NYS Insurance Fund. Once the baby is born, I'll fill in his birthday and give it to my OB. But where does it go from there? And who actually pays the whopping $170/week? My company or NYS?

I work for the state and had to have my paper work in at least 5 weeks before I went on Maternity leave. I actually haven't even got my approval back yet. They are really backed up. I filled out my part and then had to have doctors office fill out their half. I have my due date and the date I planned on going out. If I remember correctly I had to call Human resources when I had DS to give the actual birth date.

NY pays the $$.
I have to fax the form to Cigna. I think they are the administrator of the state fund. For my last pregnancy, they called me towards the end of the six weeks and then sent me a lump sum check.
