In case of medical emergency..
We're leaving Ella with her grandparents this weekend. We'll be away 4 days, so I want to leave them with some sort of authority to make medical decisions, should the need arise.
I plan on having a letter notarized - anything specific it should say?

I never considered this to be really necessary but I guess it doesn't hurt to have one written out. I'd imagine the basics should just need to be listed - giving the caregivers permission to have your child treated during an emergency in your absence.
If you sign it and it's notorized, that should be enough.

I believe it is necessary. I was just reading something about this in Parenting magazine. However, most dr. offices ask you to list who you will allow to give care to/make decisions for your DC. We signed one w/ our ped (listing both sets of parents).

My sister gave us a letter stating we had permision medically when we were watching my nephew when she went away. We had different last names than her, so whos to say if something did happen and i took him to hospital, they could think i was a kidnapper. Plus she also left me his ins. card.

Write a letter stating that Ella is the child of XXX and XXX and YYY and YYY are watching her for the weekend and have the authority to make any decisions necessary in a medical emergency.
Both of you sign it and get it notarized.

Thanks all! I found a form online that I'm customizing. We'll generally be reachable, and our pediatrician knows my parents well, but I figure better safe than sorry
(is it wrong that I'm granting MY parents the authority and not my ILs -she'll be splitting her time between them, but I want my parents making the decisions if anything happens)

Also include copies of your insurance cards with the letter. I have one that I keep in my house at all times for my Mom since she watches her 2 days a week and my doctor reminded me to put copies (front and back) of my medical cards in there.

Here's the website
I just copied & pasted into Word (rather than downloading for $10), and edited accordingly. It asks for a lot of info (drs, dentists, etc) that I didn't need to get into.

I personally think that is a GOOD idea. If you had TWO sets of grandparents who had to make a decision, it could get messy.
BTW, where are you going on your getaway?

my mom is a notary, she said it should have>>>> make sure you say like>>>

We have a letter with our doctor authorizing my brother and SIL to act on any medical care if we are unable too- such as a car accident(God forbid) or if we had to go out of town. I surprised more doctors don't do this.

We did the same and i also added my bro and SIL who live in the same state as us.
