You have to ask your employer for the forms, then you fill out a part, the doc fills out a part, then you send it back to your employer and they fill in their part and submit it. I did mine last week, which is a month before my due date.

I know people are saying to file for this right before the due date and while that is true... one word to the wise... now that the end of the year is coming, open enrollment for benefits may be occuring at your companies. Make sure you learn your companies Long Term Disability plan and ensure you upgrade your disability. I know someone who had a baby last spring, she checked with my company well after open enrollment and closer to her due date... she was quite disappointed to hear that she would only get like 30% of her salary for like 6 weeks. Had she looked into this she could have upgraded and gotten 70% for 12. You can't change your benefits outside that open enrollment period, so learn as much as you can from HR and ask questions. I'm a pain in the rear to my HR, but I want to ensure I get the most out of my time off, even if it means paying to upgrade my disability ahead of time. In fact once my paperwork comes in, I want an benefits specialist on the phone with me as I sign up.

I tried to do this with my company (after I found out I was pregnant) and they denied me because they saw my pregnancy as a "pre-existing" condition!
