For peace of mind, I did. Joseph's head was much smaller than the one that came with the car seat.
I would get the second one.

I had the headrest and NEVER used it --- I actually like the idea of the first one so much but didn't have it so honestly don't know. But I think the concept is great!

I think it depends on the size of the baby. If your child is tiny then probably the headrest might be a good idea if he above 8-8.5lbs then I wouldn't bother but I did want to get that first one but i felt it was to warm to have that since I had DS in the middle of April.

I wouldn't get the 1st one.
The second one came with my Graco Snugride, my son had a big head and we used it for a few weeks.

I had #2 and it didnt work at all. When their heads flop forward, its not deep enough to support them. When their neck is strong enough to lay back in it, you dont need it anymore. I have something like #1, not as big at all but a thick support for the neck/head like that one and it goes just under his shoulders and upper back. It works much better but not perfectly.
Do you NEED either? Not really...you can roll up a few receiving blankets and position them to probably do a better job.

My carseat(Graco Snugride) came with 2. I wound up bringing mine back(the second one you posted) and I couldn't wait to take the Graco ones out either. They made DD sweat and hardly supported her head. At 3 1/2 mos I had nothing and it worked better

I used a rolled up recieveing blanket it worked sooo much better.

I would get the 1st one, looks much more comfortable for there heads, I remember Julia's use to fall right out of the head rest, the snuzzler looks like it will keep it better in place, and looks more comfy!

I have the second one, use it with my snugrider, and the smaller one was good until recently, now her head flops forward with no support and the larger part is wayy to big.
I roll up a burp cloth(cloth diaper) to hold her head, it's doesn't work the best, but at least cushions her a little better.
