Re: Not ovulating?
How long are your cycles? Are they irregular? It is possible to get AF and not O--it's not a real AF, but rather your lining builds up until it can't sustain itself anymore and sheds. But if your cycles are on the normal side in terms of length it is possible that you just missed your surge on OPK. Either way, I'd contact the dr to get some tests run to be sure. They'll test your hormone levels at certain points of your cycle to see if you are O'ing.

Re: Not ovulating?
How long were you using the OPKS (starting on which day and ending on which day)?
I ask because the first month I got a positive, it was CD23. The previous month, I got frustrated and stopped testing around CD20, and thought I didn't O. But I just O'd late.

Re: Not ovulating?
Not all women get + on OPKs. When were you testing? Its possible that you missed the LH surge.
Are you tracking your cycles at all? Temping, checking CM, and checking cervical position are all more accurate than OPKs alone.
That being said... since you've been trying for 12 months. you may want to call your doc. I hope you get some answers!

Re: Not ovulating?
ITA with the previous posters.
The LH surge that the OPKs test for is only something like 12 hours, so if you're testing once a day its easy to miss.
Temping is a great way to confirm that you are Oing. It does take a bit of a commitment to wake up at the same time every day to temp, but I find it reassuring to see the temp spike each month to confirm that at least my body is doing what its supposed to be doing.
Maybe a visit with a RE would help?

totally agree. i think the first step to take is to start temping to confirm you are actually Oing each cycle.
