Anyone have Emblem health coverage??

DH was given an enrollment form on Friday (as he left early so we could go away for the weekend) and was told to have it back by Monday.

His boss gave him nothing with it, so I know nothing about the plan....just wondering if anyone can give me some feedback. THANKS!Emblem
