When we did one (which didn't fall through since we had issues with CO's) we used "Inspector Gadget" very nice guy and he explained everything and answered all my questions.

he saved us from buying a money pit that was about to collapse and crawled on his belly in the crawlspace to find a sewage leak. if it weren't for him, we'd have bought that place  instead of the house we're in now

We used 1800Engineer in 2006 and were really happy with him. He actually mentioned that he does not get a lot of RE agent referrals since he has had deals fall through based on his reports.
Very thorough and answered all of my crazy questions. In fact, I am planning on having him come back in and make sure that we aren't overlooking anything that may need attention.

I used these guys from a recommendation here. They have engineers on staff so ask for the engineer (not the inspectors).

We used http://www.tauschercronacher.com/ also. I highly recommend them. They helped us so much. The RE agent's face was pricely when the engineer walked in. Obviously they were hiding stuff and he found it! If you have any questions, feel free to FM.
