NY Juror Questionnaire
Back in February, I got a NY Juror Questionnaire in my married name. I filled it out & sent it in. I go to my parent's house yesterday & I got one there in my maiden name!
Do I call them & tell them I already filled one out in my new married name & give them the date????

The exact same thing happened to me.
I called them up and they took care of it.
The only one you need to fill out is the one in your married name and your current address.

Same thing happened to me too. On the one in my maiden name, I wrote that I was the same person in the 'special notes' section. A few months later, I got one in my maiden name at my new address  I had to call to straighten it out!

That's right. If you ignore it once, they will send you another one and that one will say that if you don't send it in, the next step will be being summoned to court.
Just know that once you get that and send it back, you will be getting a juror summons within a few months....

Every time I changed my name, I got one..... I was so fustrated. I ended up sending back both and put in a copy of my marriage license all HIGHLIGHTED and a letter, HIGHLITED, and a copy of the receipt for when I served HIGHLIGHTED. They must have thought I was highlighter crazy!!! LOL I guess it worked I have not been called since then a year or so ago.... ( Knock on wood)

Yeah...My DH got one and didn't fill it out and now he has to appear with a letter they sent him....He's a dummy!!
