NSUH pre-registration forms
Did anyone who delivered at NSUH have to fill out pre-registration forms? I haven't gotten anything from them and I don't know if there's something I'm supposed to be asking for in advance. TIA.

I am Delivering at LIJ and my Drs Office gave me that PreRegistration Form like within the First few visits .. I havent of course Filled it out yet But you Dr's Office should Def hav eit

I delivered at NSUH and my doctor's office gave me the pre-reg forms. I think around week 26ish they gave me the forms.

me too! I am due this tuesday and I received the folder of forms when I got the glucola for the glucose test. I would ask your doctors office or call NSUH directly.

I was wondering this also, since I'm 32 wks and haven't gotten anything yet. But, I guess I will ask when I go there for my Lamaze classes/hospital tour

I got them on my first visit to the OB, and I'm pretty sure they told me to mail them in by 7 months...I'm just as happy to get them off my hands, but I think I'm going to double-check with the receptionist when I go back on Monday. I'm sure if I bring them with me when I'm ready to deliver, they won't turn me away.

My OB provided a folder with NSUH information and pre-reg forms. I mailed in what was required.
