I wll be married 4 years in October and I JUST changed my name because Im due in September and I wanted us all to have the same last name. It wasnt as big a hassle as I thought it would be. The DMV and the SS office were pretty fast and then I just had to give a copy of my marriage certificate for my work info and insurance to be changed. it was really hard for me to take this step because like you, I had to change everything at work. Im an attorney and Ive been practicing under my maiden name for almost 8 years so it was really something I was dragging my feet on but to me I just thought it was more important for us to have one family name so I finally bit the bullet.

I did change my name before ds was born last year and his bc only says "mother's maiden name" regardless. Obviously, this is not always the case since different ladies here are reporting different things while looking at their dc's bcs. So, yeah I guess it was a good call going to change it so that u can be certain. As far as the bracelet/name on bassinet in hosp I know recently my friend had her maiden name on that stuff bc that was the name on her insurance.
