If you have brought a babysitter on vacation, how does it work?
I know that you pay for their vacation, flight, etc, but in regards to paying them, how does it work?

I have never done it, but I would assume you pay a flat amount, certainly not by the hour. Maybe $300-500 for the week? And I would also give her some down time during the day and/or evening so she didn't feel she was working all day and night.

That's why I am not sure if this is something for us. We like spending time with our kids, but were thinking of it for maybe a few nights just to get out...
If anyone has done this, what are your experiences?

How about a cruise ship? You can put the kids in camp for a couple of hours here and here so you can have alone time?

Some cruises offer group babysitting, some will do it in your cabin.

We ended up booking Beaches Turks and Caicos instead of trying to bring a babysitter. They have a supposed awesome kid's club.
If we were to bring our nanny or other babysitter, I would offer flights, room/board and probably pay for certain hours, the same way I would pay if we were home. I would also offer to pay for all her meals when she is with our children but I would expect her to pay for her meals when she is not working.
