If DS doesnt have an accident or something, I change it once a week. If he is sick (coughing / runny nose) I change it every other day.

As little as possible.  Changing the crib sheet is right up there with my most hated parenting chores.
ETA...because of the bumpers, the aquarium and the positioning of the crib. HATE IT!
You need the Ultimate crib sheet! I swear by it--you don't need to move the bumper or aquarium, and it just snaps around the sides of the crib!
I love those. I change them once a week.

Every other day because she drools a lot, unless she wets or has a cold than every night.

We also have the ultimate crib sheet so it is very easy to change & wash. We try to do that 1x a wk unless she has an accident before or drooled a lot etc...she never sleeps on the sheet directly & it is covered by the uktimate crib sheet so we don't change the actual sheet often.
