Do you do this?
When you call someone, a company where you do business, do you start your conversation off by telling the person your name and any other pertinent info?
This is a huge pet peeve of mine. Clients call and start rambling on about their contracts, yet I have NO IDEA WHO THEY ARE!!!!!!!!
So after all the rambling, I have to now ask them, WHO ARE YOU?!?!?!?!?!?!?!?!?
And then start from scratch again.

Same here. IMO it's common phone courtesy to identify yourself before you ask for someone or for information.

i always state my name and give a little background (hi its emily from so and so, we met last week at XXX). if not, how will people know who the hell i am!
you know what else i hate...when people call me and leave a voicemail and say "hi its me" and then start leaving a message! hello...who the freak are you??? my cell # is on my business cards so unless your # is saved on my phone, i dont know who you are cause so many people call!!!! i have to call back and be like "hi, someone just called me from this #" and then i feel bad!

I always identify myself. I do have clients who call me and just start talking, but there are many whose voices I know. If the call comes up as private or out of area, I double check who it is before I say too much.
ETA: My office phone number is very similar to the phone number of a gyn. office. I get a lot of wrong number calls from their patients. I cannot believe how many calls I get where the person starts rambling about test results for things that they probably do not want some random person to know about.

No matter where I call, even if it's for the first time, I always introduce myself first before I get into the reason for my call. Just seems like common courtesy to me.
