Dementia Care & Short Term Rehab?
My grandmother has had a live-in caretaker for months but she's been trying to escape/flee, so she needs to be in a dementia care facility.
We are looking for a long-term care facility (private pay) for her on LI, but in the meantime she can do a short-term rehab facility covered by Medicare for a month while we find a long-term care place for her.
The hospital she's in now said she could go to Medford Multicare but the ratings by Medicare.gov are the lowest in the area for that one.
Does anyone have experience / referrals for either type of facility (short or long-term) with lock-down for dementia patients?

The only facility I've dealt with was St.Johnland in Kings Park. It wasn't for dementia but for inpatient therapy do to a TBI injury. This was over 10 yrs ago. We were very happy with the care. But I know they also have dementia residents there. Not sure about the Medicare portion though.

My husband's aunt was in Broadlawn. It was OK. I think pretty much all the nursing homes offer dementia care, so if you search for that you should find something. Good luck!

This is where my grandmother is and they've been great. She did short term rehab there in the past and has been there full time for over two years now.

Really nice place
We recently got FIL placed in Mills Pond in St. James for long term. Nice place, I liked Huntington Hills better but they didn't have a bed and we needed to move him from the place he was in. Next door to Mills Pond is St. James Rehab & Health Center, also very nice. They also didn't have a bed for FIL. Did not like Smithtown Center for Rehab - they let us look around without an escort. I could have walked into any resident's room.
eta : Mills Pond is $460 per day private pay, that is for a shared room. Not sure if dementia unit differs.

My mother in law was in The Amsterdam at Harborside in Port Washington. Really nice place and staff.
