Five feet may not seem like much but it could be a lot; it may benefit the seller to add that space to his property. I assume the five feet will run the length or width of the property? I agree with the poster who said you should get your own survey.

you can definately get your own survey, and may be what you need to do for your piece of mind. do you have the survey from when you bought the house? they very easily could have a correct survey, but still built over the line. things were pretty loose in 2004.
once you decide that the neighbors survey is correct (if it is) you're stuck with either signing the form, or taking down the shed. If you don't sign, they won't be able to close until the shed is down.
the form is basically protecting the title to the neighbor's land that you are using and not allowing your encroachment on the boundry causing an abandonment issue on their side.

I would get my own survey ASAP unless you believe the neighbors survey is correct or dont mind giving up the 5ft. What if the new neighbors decide to put up a fence?

I would speak to a real property attorney. I believe there are also "abandament" laws that might help you keep the property.
