Just to follow up too.
She called me back today (almost 24 hours later) and AGAIN asked me what date I needed it to say.
Then I asked if she was working on the forms now and she made this gasp and was like "ooh no, its almost time to go home, I will deal with this tomorrow"

You know, down the line I bet you could write one hell of a memoir about this experience. I bet it would be an incredible read- from the dealings with idiots like these to your post about things to leave behind- it would be really touching but also humorous and inspiring.
