Quite Honestly - I think the whole point of having that in there - was to gross us all out
I thought it was just that she was grasping at straws to find something to get a reaction out of him. My grandmother was in a coma and after a while we all started saying really stupid or weird things just to see if that got any sort of reaction. You start to grasp at straws after a while.
