maternity leave questions
Is there paperwork I need to fill out in order to recieve state disability??
What about to be covered under FMLA...
Im the first person to go on leave at my job, and Im the one who would generally handle these things... so I dont know where to go about getting the info...

There is a form that has to be filled out for NY State DIsability and there is a section your dr has to fill out. I am not sure of the form but I sure you can google it.
For FMLA I don't believe there is a form you would just have a cause of action against your employer if you were out for the 12 weeks and didn't have a job upon your return.

How do I get these forms?????
My c-section will be scheduled next week... so I wanna try and take care of as much paperwork in advance as possible... (so that the checks start coming in as soon as possible)

This isn't the same form that I was given at my job, so I'm not sure, but this might be helpful.

You need to fill out Disability form 450 (DB-450) - I believe that that it what it is. You fill out section A and your OB fills out Section B. Get the name of your insurance carrier from your employer, fill out Section A and then give both forms, with a copy of your W2 to your OB with a stamped addressed envelope to the insurance carrier. The OB's office will have the dr. fill it Section B and then mail it on to the insurance company. This is the information that was given to me by NYS Disability and that is what I have done up tp this date. FM if you have any further questions.
Also, I did not fill out any forms re: FMLA per se, just the NYS Disability form.

I am in the same situation as you & I got the form...The NYS form is NY DB450. If your company has a Disability company handling it, call them (we have The Hartford)
They will talk you through all the protocols.

I'm a teacher, and at the DOE we get 12 weeks maternity leave and six weeks disability if it's a vaginal birth and eight weeks for c-section. CHeck with your HR dept. HR should provide you with everything you need. If you need disability I would ask the receptionist at your dr's office, they may be able to give you info or a web page.

What kind of insurance carrier???
Will this effect the seperate paid time off I have through my employer or the seperate STD plan I HAVE BEEN PAYING INTO????
