Anyone by chance registering their children for K in the Lindenhurst school district? I have a ?
I've called the central registration office a few times and no one answers nor have I gotten a call back. Registration is this week and next week and I need to know if the health appraisal form and the immunization records are needed at the time of registration? My twins are not due for physicals until the summer so I'm not sure if I have to pay our copays to take them for their physicals early or if it can wait until June for these 2 items? Does anyone know? TIA!

Not in that district but can tell you we had the same issue. DD wasn't due for a well visit until the summer.
They said just as long as you have the forms before school starts ,it was fine.
Your insurance won't let you do 2 well visits in a year so if you went now you'd have to pay out of pocket.

Thanks! Did you give immunization records for what she has completed now? There are still shots my kids have to get before school that they will definitely get over the summer. That I can have them email to me but I'll just have to provide an updated one anyway.

I gave them a copy of what we had already- then they wrote down what was needed before school started so I could take that to my ped for her well visit and complete the shots

Not in that district but in ours, their last physical had to be less than a year ago and updated records needed to be forwarded to the school once they were received.

I'm in Lindenhurst & this is exactly how it was for me when I registered my son last year. His birthday is in Sept. and he wasn't due for a well visit until the end of the month. They let him in with the records from the previous year and I then had to send in the updated copies after we went to the doctor.

Thanks everyone! I emailed the registration office. Hopefully I'll get a response this way. I am really hoping they'll let wait until they've had their physicals to submit that form. I really don't feel like driving to smithtown to our ped to drop off and then pick up 2 pieces of paper.
