Yellow immunization card
When dd got her 2-month immunizations yesterday, I was surprised to learn that the doctors office does not use (or even have) the yellow immunization cards. They provided me with a digital printout of her immunization data, but I'd also like to keep the written record in the widely-recognized yellow card. Any idea where I can get one? Thank you.

I have never heard of a yellow immunization card.
I don't even have any record of DDs shots.
As long as the ped has it in her chart that's all i am concerned with.
I can't see ever sitting down and reviewing it in my spare time. I know she is getting what she needs because the ped keeps track

I have them for my twins. I guess its old school. Maybe google the layout of it and make your own?

Yellow immunization card
Mine was given to me in the hospital. But when i asked the pediatrician office to write the weigh and record they said they will just gve me print out and i am the one who will write down on my yellow card.
My previous pedia office used to write it down for me.

My kids ped gave us a book at the first appointment and after they record the official info they will write it all in our books. I bring it for every annual visit and my kids are 5 and 3.

I got mine at the hospital also.
Yea me too. And my ped asked me for it at her first appointment...and her second appt as well.

I am just a little unclear on what the point of this card is? In this day and age everything is inputted into the computer at the peds office.
When my daycare needed records of her immunizations I had my ped fax them a print out and they put it in her file. They wouldn't have accepted a hand written card...after all, I could have written in whatever I wanted on it. How would they know it was legit?
What is the purpose of this card?
Seems like something from a past era...before computers.

I got it from hospital too. My dr fills it in for me each time I go. I like it because I have a record at home of all my ds shots, height,weight,ect since birth. I know my dr has the info but its nice to have it too.

Something as serious as immunizations, I want a further record than just computer. What if there is a domputer crash or bug? Plus, when my child is in her 20s and may need to know the shots/dates for a job or foreign travel, what if by then her ped office is closed, or has archived information, or.... I just want a backup.

Yellow immunization card
I never got a card. It's in the file at my peds office. That's all I need.
Just ask the Ped for a copy when the child gets new shots. It doesn't have to be a card. It's not like you are less likely to lose it or something cause its a yellow card.

It would all be is NYS's vaccine registry anyway, so if you neeeded your records to be accessed and coulnd't get it from your ped you'd call them. Any vaccine ever adminstered to you child is on record.

Yes i just feel like in this day and age relying a piece of paper is worse than relying on computerized records.
I'd probably lose that card anyway... I lose everything.
Not even sure where DD's birth certificate is at the moment...

For each of my kids, I have the book from Enfamil and our ped. provides a printout. I'm not sure I ever got a yellow card.

Yellow immunization card
we got ours from the hospital and out dr puts down her measurements, weight and vaccinations.
I keep the card in my diaper bag so i never forget it (thats the tips the hospital gave me)

Our doctor gave it to us at our first appointment. The nurse asks for it during every appointment and fills in his stats and the shot info.
