i filled out the forms given to me by my OB's office and mailed them in.
Surprisingly enough they had everything there when I showed up. I had heard that a lot of times pre registering is a waste because they make you fill out everything anyway but I guess I lucked out

My OB's office gave me the forms and I just brought them back there after I filled it out.

A few months ago, my OB gave me the papers to fill in. It was only two sheets I think. They then sent them to Winthrop for me.
