So remember how well I did with the Mommy Makeover Challenge
I had lost like 15 lbs, could run a mile, quit smoking....yeah, Ive put all but 4 of that back on, stopped running and exercising and started smoking again.
I really want to get back on the horse, but Im so discouraged and I NEVER keep at it, EVER, for longer than like 8 weeks when I do stuff like this.
Any suggestions?

My advice is to not try to do too much all at once. It sounds to me like you were trying to make a lot of changes at the same time. Focus on quitting smoking first and at the same time as that make some minor changes to your diet. After about a month or two with your body accepting of those changes start introducing an exercise program that fits your schedule. Don't do too much at first and slowly ease yourself into everything.
Finally, don't think about what you've done as a failure. Take it as a lesson and build yourself from there. Good luck with it.
