I have connected with former grad professor on Linked In. Since that is more of a professional networking site, it may be more appropriate considering your relationship - if you are both on there, that is. If not, I guess if he sees it as innapropriate he won't accept.

As a grad school instructor, I would be okay with my students requesting me, but I might limit the personal information that they are allowed to see.

Considering all of the circumstances, I wouldn't find it to be inappropriate at all. As some of the others mentioned, send a note along with it  I'm a teacher and I have parents on my page! Nothing wrong with that!!
