i am almost in the same situation as you...except my appt is in May and i have only been ttc for 7 months.
I plan on showing my doctor all my charts (temping, and also how long my cycles are and when i o). I am going to ask her to do a full blood workup and also ask what the next steps are in terms of testing for any problems. from what she told me months ago though, the first step would be having DH go for a sperm analysis. If you can have your dh go for one before your dr appt that may be helpful bc it can rule out that the sperm quality/quantity is the problem and your doctor can then focus on you.
