Help! I am meeting with at At-Home Day Care....

Help! I am meeting with at At-Home Day Care....
on Saturday. She has the best rate in our area and sounds the most professional.
What kinds of questions should I ask her?

Talk about pay. Set rate per day or is it an hourly rate. Do you pay her on days that your DC doesn't go, such as when they are sick, or only on the days that they go?
Do you need to send in meals/snacks?
Will there be other kids there who have food allergies, in case you ever send anything in?
Other kids in the center? Ages? How many people to help and are they regular, consistent people?
What kind of "extra curricular" activities will they do at different ages for the child. Reading books, crafts, walks around the neighborhood, etc.
Any animals to be aware of?
Policies as far as sick children? Runny noses, fevers, how many days after a fever before they can return, info that is helpful not only for your child but in case another child at the center is sick!
Early morning hours, late night hours? What happens if you are running late or hit traffic? Will you get charged extra?
Will your daycare provider have any vacation days worked in? Do you pay them if THEY call out sick and you are unable to take the child?
Just some questions off of the top of my head. I babysit for a little girl from my home so those are some of the things that we have discussed.

Just thought of another one ---
Do you need to provide wipes/diapers? Do they want them sent in on a daily basis (ex 6-7 diapers per day and a travel pack?) or large packs at a time?
What about change of clothing? Do you send in an extra set each day or will she keep one on hand at her house?
