We get a form at about 24 - 26 weeks that we give to HR along with a letter saying that we're pregnant (SURPRISE!) and the baby is due XX XXX and when you intend to go on maternity leave. I gave my form to HR a few weeks ago, but only managed to get her the letter on Monday. She wrote me back confirming everything a few days ago and the ad for my replacement is going in the paper this Saturday!

Only 8 more weeks of work left to go!

I contacted them 3 months in advance, but they said to hold off on filling out the paperwork until about a month/month and a half before the EDD. I'm really glad I waited--I had thought I could go til I give birth, and it's becoming increasingly difficult, so I think had I filled out the paperwork I would need to do it again anyway to change my leave date.
