Let's vote...
DH just called on my train ride home to let me know that Maddie's cleats were not in the soccer bag for practice.
I just sent him the following text message:
"I'm sorry about the cleats. I completely forgot to check that she had them in the bag.
I was more wrapped in getting the laundry done for picture day, going to my dr appt, cleaning up from dinner and grocery shopping for their lunch last night. My fault completely."
How many of you think he'll get it??

The funny thing is this am, while getting the kids fed, dressed, making lunches, finishing laundry, filling out permission slips and trying to find my checkbook for the photos, I saw the soccer ball by the couch and thought "he didn't put the soccer ball in the car?" And considered how to arrange to get him the soccer ball before thinking I'll put it in the middle of the living room so he didn't miss it. But nope completely forgot she didn't put her cleats in the bag.
He was watching TV last night while I ran around and had to get himself dressed and out of the house. I'm sure the ball and cleats didn't enter his mind once.

Now you mentioned he had to dress himself. Did he match? Because if he did then I can totally see how that took all his focus.

Honestly I HAVE TO put stuff together myself. I seem to be the only one who can think what a "going out" outing entails.
I seem to be the only one who think the all thing through so we don't forget anything whether it's food related or the camera.

And don't forget... "calling your friend to wish her a happy birthday as you were running to your doc appointment" 
Seriously, he's a big boy. Tell Snuffie to use his "eyes" and double check the bag before he leaves.
As you know DH travels a lot, years ago he made a laminated travel/packing list that he keeps in his suitcase to make packing a no-brainer. Maybe Snuffie and the kids need one for each of their bags
And I agree with a PP, a lot of men would take that as an apology
