How do you go about pre registering at the hospital?

My doctor had me fill it out in her office about a month and a half ago and said she will take care of the rest.

I just did this on Saturday. My hospital had the application available on the website, so I filled it out there.

In case you need to get in earlier than scheduled and are already in labor. The last thing you will want to do is filling out the paperwork while you are having contractions.
To the OP, my dr. gave me the paperwork to fill out at the very beginning of my pregnancy. I had not filled my out yet (proscratinator) and I had a false alarm at 34 weeks. Because I did not fill out the paperwork (I thought I had plenty of time!), I had to do it when I went to the hospital at 34 weeks.
So in essence I was ready when I had DS at 39 weeks.

At SB hospital, you just have to mail in a postcard with basic info about 2 mo prior to the due date... I don't know if they mail you a packet to fill-out in response to that card though

I got a form from my OB. Filled it out and mailed it to St. Charles.

I got one in response to the postcard (last year). I was a step ahead because I had also gotten one on my hospital tour.
