Underwear when I'm talking about it for real (like...laundry?), but underpants when I'm talking about it in a funny way...as in, "I can't believe I put my underpants on inside out again!"
I think the word 'underpants' is hilarious for some odd reason.

I use both terms depending on the situations. I also like, but would never use, the English term "Knickers" as I think its hilarious. Unmentionables is an adorable term used in Victorian times.
