Name your mattress!
Obviously, I'm working on our registry today... thus all the questions. 
What mattress are you going to use?
Is it Spring Coil or Foam?
and
What mattress cover are you getting? I've seen everything from $12 to $70!!! I'd rather not register for 2-3 $70 mattress covers!

Our mattress is the Visca Classico II:
Our mattress pad is the bb Basics Waterproof Fitted Crib Pad:
The mattress we picked was middle of the line ... didn't want to spend too much or too little. IMO, a mattress pad is a mattress pad.

Sealy Soybean foam - $95 on amazon.com and it comes covered in a protective mattress cover already although i have sheet protectors to go over the sheets if i so wish as well.
