Thank you etiquette question
we received a gift for DS from SIL's best friend.
It was a truly sweet gesture, except that the friend sent us monogrammed shirts with DS's first name and SIL's last name. SIL has a different last name than we do.
Should I just send a TY and ignore the mistake or make a joke out of it like "gee, I knew we weren't the best of parents, but is DS being taken away from us already?"

ITA. It was a nice gesture. If you like it, maybe there's a way of removing the last name somehow?

Re: Thank you etiquette question
I would just send a thank you for the thoughtful gift. People get weird about gifts and thank you's.

ITA with the others. Just send a thank you and ignore the mistake. She'll probably realize it when she sees your name on the return address..

That was what I was going to say...just focus on the gesture in your thank you...she'll probably realize it right away.

Re: Thank you etiquette question
I definitely wouldn't say anything in the thank you card (words on paper can be totally misconstrued), but if the right opportunity arose, I might joke about it in person.
Edit: I just reread and saw that it was your SIL's best friend (NOT your SIL as I previously thought) - in that case, I definitely would NOT say anything, ever! (not even joking)

I wouldn't mention it. It's the thought that counts and a personalized gift is extremely thoughtful.
I wouldn't even put my last name on the return address. I wouldn't want to make them feel bad.
