What form do I need at DMV to change my name?

OK yes, almost 2 years later I am finally going to get this done. I want to have the form filled out before I go though, I am not sure which one to print off the dmv website. Is it the regular application for a license?

Also, does it matter if I go to DMV before social security?

i am pretty sure you need to get your new SS card first, but that only take a couple of weeks once you submit the proper forms. there are several forms you need to fill out a the DMV, i think all i had to do was a name-change form and a title-change form.

How to Change Information on a Vehicle Registration or Title Certificate
Learn how to change your address or your name on your vehicle and driver documents and records.

To change information except your address or name shown on a vehicle registration or title certificate, bring the following items to a DMV office:

A completed form MV-82 (Application for Registration or Title).
Your current title certificate or other proof of ownership and the registration document.
Proof of identity and date of birth. If another person brings your application to the DMV office, that person must follow the requirements for a second party application for a registration.
Cash, personal check or credit card to pay the fee.
An Insurance ID Card (FS-20) in the name of the registrant.
Proof of the change. For example, to change the vehicle identification number, bring proof of the correct number from the automobile dealer or manufacturer. To change the unladen weight of a vehicle, bring a weight certificate from an official weigh station.
Unless the change affects the registered weight or registration class of the vehicle, there is no fee to change information on a registration or title certificate.

I dont know the name of it but if you go on their website it will direct you. It will save you time if you fill out the form before you go down there , its online

FYI, since you are changing your name they make you take a new Picture. I was unaware of this & look downright frightening on my new license

Thanks! I did know this, lol.
When I looked at the forms of ID needed, you don't need a SS card, so does it really matter if that comes first?
I need to get the new lic. done right away so we can apply for STAR by March 1.

i just did this a couple weeks ago... you do not have to go to SS first... i went to DMV, printed out the form from the dmv website... with your marraige certificate.. (the original) and thats it!! and yes you will have to take a new picture!

Change Your Name
You must visit a local DMV office to change your name on all DMV records and documents.

Bring your NYSDMV photo document or other proof of identity that displays your previous name and has a value of at least six points.
Bring a US marriage certificate, US divorce documents, or other court papers issued in the US that prove your new name. Or, you can bring proof of identity that displays your new name and has a value of at least six points.
To change your name on a registration document and a title certificate, you must bring the registration document and the title certificate that display your previous name.
Use form MV-44 to change your name on your driver license, learner permit or non-driver photo ID card. There is a fee of $15 to get a new photo driver license or learner permit document that displays your new name, or $8 to get a new non-driver photo ID card. Your new document arrives by mail in approximately four weeks.

Use form MV-82 to change your name on your registration documents and title certificate. There is no fee to change your name on your registration documents and title certificate. The DMV office gives you new registration documents that display your new name. A title certificate that displays your new name is mailed to you.
