What are your diaper bag must-have's?

I carry around way too much crap.
Other than diapers & wipes I have... a change of clothes for each kid, a sweater, sunglasses, sunscreen, hats, toilet cover for public restrooms, sippy cup/bottle, bib, a few plastic bags for garbage or dirty diaper, tissues, a placemat & spoon for if we go out to eat, snacks, lotion/bacitracin if someone gets a cut, Epi Pens, stuffed animal/book/toy.... probably stuff I'm forgetting.
I take almost all this too, but not all of it would fit in my diaper bag. What bag do you have?

I try to pack it in buy usually... Half the stuff is in my diaper bag, half is in my handbag  I don't have a really big diaper bag just a regular one.
