Those who have had an evaluation for their child through the school district?

My sons preschool teacher recommended a speech evaluation for my 3yo son. I decided to also have a speech evaluation done on my 4.5 year old son as well and a psych evaluation for my 4.5 year old daughter. My ped mentioned having speech evaluations done for the boys a few months ago and gave us "scripts" to call EI. I decided yen to wait for parent teacher conferences to see how my kids were I school. When I asked my ped about some behavior issues with my daughter she said I can have a psych evaluation done but didn't give me a "script". Anyway, I called the school district yesterday and they sent me the paperwork today. On the paperwork it asks for a signature from the person referring my child(ren) for the eval. I can get that from my youngest sons preschool teacher since she recommended it. But for my other 2, do I need this signature? I can get it from our ped but their in smithtown and I'm in Lindenhurst and I'd hate to have to drive all the way there just for two signatures.....they'll most likely have me leave the forms and pick them up when done. Does anyone know if I need this signature on the paperwork?

I just handed over all of my DS paperwork to our school district the other day. I don't know if your district has different requirments but I had to have his ped fill out the school districts paperwork with his medical history plus sign. I sm the one who initiated the eval so I signed. Are you the one who originally called the district to inquire about the eval? If so, than you should sign. If your still not sure, just call the contact you have at Student Services and double check. GL

I signed that stating I was initiating the evaluation for my DS. I did have to bring things for my ped to fill out though, so maybe double check all the paperwork incase there's other stuff to fill out, so you could have it done all at once.

I initiated the Eval (PT) for my child so I signed it.

Parents can refer their children for testing, so you can sign.

Wondeful, thanks everyone! We need a letter from our ped stating oirblast physical, but I am going to have them fax that to DH on Monday so I can bring the paperwork back on tuesday!
