Cultured Stone

Does anyone have any idea how much this would be per sqft including installation...or have any recommendations on who to call? I am looking to do something like below for my fireplace (cathedral ceilings)

I have a double sided fireplace that we did this to, we have 9 ft. ceilings, so not the cathedral. It cost us around $2000 per side including supplies I believe. I do know the stone I picked was also a special custom order which added to the price per square ft and the time it took to get the stone. I have 1 pic in my house photo album and that is before we hung the mantel.
We used Jeff from Perfection located in Hicksville. He did all our outside stonework as well (cultured stone on house, driveway, pathways, and patio.)
