401K Spouse Beneficiary
There's a part on the form that asks for Spousal Consent related to Qualified Joint and Survivor Annuity. Does anyone know more about this? Did you have your spouse sign it?
It seems like he should not, I've never had him sign that in the past.

I believe you only have to have him sign if you list someone else as your primary beneficiary.
I'm not 100% sure though.

There are a couple of times your spouse would have to sign like this.
as said before, if you want someone else to be your primary beneficiary
or
if you wanted to take a loan or withdrawal
