Sasha Alexander who played Kate was killed because the actress who played her wanted out. She couldn't handle the working pressure of weekly television, and she didn't want to be on a show that could possibly be on the air for 5 years (I think JAG's 10 years also got her worried). She asked to leave the series, and out of her contract. I think she thought that this was something she didn't want to do.

Me too. I was hoping it was a day dream in Gibbs head and that he was going to wake up in front of his computer at his desk and Kate would be handing him coffee and saying "rought night boss"
